Three steps to a good request:
1. In the first (smaller) text box, please summarize your issue with a meaningful "subject/headline".
2. In the second (big) text box, please indicate which version of MediaWiki, PHP, and MySQL database you are using. Then explain the issue. Provide as much information as you can.
3. Scroll down, click "Show preview", check what you wrote, and then click "save".
